All payments are final and must be made to The Medical Skin Clinic prior to the performance of services. Membership fees must be paid via credit card but can be paid as one annual payment. Prepayment of membership fees must be paid in yearly installments and is available for a maximum term of 3 years. The membership fee is $149 per month. There is a one time enrollment fee of $50 paid only once as long as the membership remains current. Monthly membership fees are drafted on a 30 day rolling basis and are typically withdrawn on the same calendar day each month.

The Medical Skin Clinic memberships are for a 12 month minimum term, which renews automatically each year unless the notice below is provided after the 12 month term. After the initial 12 month term, the membership becomes month-to-month. The membership starts the day you sign up for the membership and renews on that date each month. Member discounts and benefits renew on that date as well. Adjustments may be made during leap years. The unused membership benefits do not carry over from month to month and are forfeited if they are unused.

To cancel a membership after the initial 12 month term, a 90 days in-person notice is required. The Member must cancel the membership by coming into the office in person. Please make an appointment at The Medical Skin Clinic location as memberships can only be cancelled in person. To cancel a membership prior to the end of the 12 month term, the Member must have moved 50 miles away from any The Medical Skin Clinic location and demonstrate this move with a utility bill (electric, gas, or water) in the Member’s name at the new home address. All cancellation requests due to moving must be sent to info@TheMedicalSkinClinic.com with the supporting documentation attached. The membership will then be canceled at the next billing date, provided accurate supporting documentation is attached.

On certain designated membership anniversaries members may be eligible to receive treatment gifts, as may be specified in the membership brochure or materials from time to time.

Membership pricing, promotions and offers are non-transferrable and are for the Member’s use only, unless otherwise noted. Membership discounts do not apply to training, modeling and special flash sales. Monthly Membership dues cannot be supplemented by any other promotional vendor programs such as Alle or Aspire Rewards, but if a Member for example purchases Dysport, they get the member rate, plus they can use their Aspire points.

If any membership fee is returned for insufficient funds, incorrect card number or information, it will be considered in arrears and due to The Medical Skin Clinic, plus a $50 reinstatement fee. Membership services and discounts will not be available for use until balance is paid up to and including the current month due. Membership fees cannot be offset by a store credit and must be paid by Visa/Mastercard/American Express/Discover. Membership funds and/or the balance of a member’s virtual wallet or prepaid treatments, cannot be transferred to the virtual wallet of another person.

This Agreement shall be construed under and governed in accordance with the laws of the State of New Hampshire.

Any dispute arising under this contract shall be resolved under the commercial arbitration rules of the American Arbitration Association.

A Member shall not hold The Medical Skin Clinic responsible for other Members’ actions or inactions.

Membership payments are non-refundable.

Particular appointment availability or provider availability is not guaranteed. All of The Medical Skin Clinic providers go through a training process. Members must abide by all The Medical Skin Clinic policies expressed in this Agreement, and must arrive on time to their appointments.
